Trump Administration Sends $90 Medicare Rebates to 20.8 Million Seniors Four Weeks Before Midterms

The Trump administration is sending one-time $90 payments to 20.8 million Medicare beneficiaries this week, with the money arriving just four weeks before the November 3 midterm elections.

The payments are designed to offset part of the cost of Medicare Part B, whose standard monthly premium is $202.90 in 2026. Most eligible beneficiaries are expected to receive a $90 direct deposit around October 8, while others will receive Treasury checks later this month. CMS says recipients will also receive an email or letter from President Donald Trump in mid-October.

The payments come from the Medicare Improvement Fund, a $2 billion fund created by Congress in 2008 to support improvements to the traditional Medicare fee-for-service program. The administration says the fund had never previously been used to make direct payments to Medicare beneficiaries.

At $90 per person, the payments total roughly $1.87 billion for the 20.8 million eligible beneficiaries.

Not everyone enrolled in Medicare qualifies. The rebate is available to people enrolled in Original Medicare Part B who live in the United States and are not receiving premium assistance through Medicaid or paying higher premiums under the Income-Related Monthly Adjustment Amount. People enrolled in Medicare Advantage are excluded.

That means the payment reaches less than one-third of all Medicare beneficiaries. More than 35 million Medicare recipients enrolled in Medicare Advantage, along with other groups, will not receive the rebate.

The administration describes the program as a way to make healthcare more affordable. The White House says the money is being returned to Medicare beneficiaries rather than left unused in the fund.

The timing has nevertheless attracted political scrutiny. The payments are arriving as Republicans defend their congressional majorities ahead of the November 3 elections and as healthcare and the cost of living remain important issues for voters. The $90 rebate follows other payments and proposals promoted by Trump, including $500 Affordable Care Act-related refunds for roughly 1 million people and his proposed $5,000 “Trump Dividend” for adults if Republicans retain control of Congress.

Trump’s broader campaign has increasingly emphasized direct financial relief as Americans continue to face higher costs. The Medicare payment is different from the proposed $5,000 dividend, however: the $90 rebate is an announced and funded payment for a defined group of Medicare beneficiaries, while the $5,000 proposal would require congressional action.

For the roughly 20.8 million people who qualify, the $90 payment will cover less than half of one month’s standard Part B premium. The first deposits are arriving around October 8, with presidential letters or emails expected to follow in mid-October.

The result is a highly visible government payment reaching millions of older Americans immediately before voters head to the polls—a fact that has made the rebate part of the broader political debate over healthcare costs and the 2026 midterms.
